OAK Orthopedics will be conducting physicals for students entering fifth (5), seventh (7), eighth (8), tenth (10), eleventh (11) and twelfth (12) grades on Saturday, August 7, 2021 from 9-11 a.m.
Cost for the physical is $20.
Students should bring the 2021 IESA/IHSA completed form with a parent signature on the form.
Students should dress in tank tops and shorts.
All participants must wear a face mask/covering.
