Employee Leave Request -  This is the employee leave request in Microsoft Word Format.  Please double click on the link above.  When the window pops save this file to your desktop.  Then open the file for editing, save the completed file.  Finally attach and e-mail this completed form to your supervisor.  This should speed leave request processing.
